PHOTOGRAPHY WAIVER RELEASE:
I (who am the participant’s parent or legal guardian if the participant is under 18 years old) hereby grant and authorize the Cst. Rick O’Brien Joint Forces Jamboree the right to take, edit, alter, copy, exhibit, publish, distribute, and make use of any and all pictures or video taken of my Minor(s) to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits, and promotional video,, websites, social networking sites and other print and digital communications, without payment or any other consideration.

WAIVER, RELEASE AND INDEMNIFICATION:
Upon Signing this waiver agreement, I (who am the participant’s parent or legal guardian if the participant is under 18 years old) forever generally and completely releases and discharges and shall indemnify and hold harmless Cst. Rick O’Brien’s Joint Forces Ball Hockey Memorial Jamboree and its owners, employees, directors, officers, agents, sponsors, partners and licensees and each other affiliated with Cst. Rick O’Brien’s Joint Forces Ball Hockey Memorial Jamboree and each of its owners, agent, directors, officers, employees, sponsors and partners (collectively, “Event Organizers”) against and from any and all claims and demands of every kind and nature whatsoever, for damages or injuries, actual or consequential, past, present or future, arising out of or in any way related to the Cst. Rick O’Brien’s Joint Forces Ball Hockey Memorial Jamboree, including but no limited to any claims of injury from participating in OR observing the jamboree, the loss of personal property by theft or acknowledges that of its own free will he/she has chosen to participate in the jamboree and related activities. I understand and acknowledge that the aforementioned activities carry with them a level of risk of injury and additional hazards related thereto.

2024 Cst. Rick O’Brien Joint Forces Jamboree Registration $30 This $30 donation is going to The Cst. Rick O'Brien Joint Forces Foundation so we can continue to create inclusive activities, events and programs to foster meaningful relationships between youth and law enforcement for positive change.
